Have you tried using a different yeast.
Specifically have you tried using liquid yeast?
Second, do you ferment in plastic? Possibly you
have some nasty living in your plastic fermentor,
switch to a 7 gallon glass carboy or buy a new primary
and then becomea fanatic about sterilization. If you do
use a plastic primary, use it for beer only, don't put
anything in it that could scratch the plastic.
